YOUR FIRST
OFFICE VISIT
Your initial office appointment
will consist of a consultation with your surgeon to evaluate
and determine your diagnosis and treatment options.
Simple extractions can often be performed in conjunction
with your consultation. However, complex extractions or
difficult medical history will require a second appointment to
provide treatment. IV sedation or general
anesthesia may require a consultation appointment, as
restrictions apply.
If your referring dentist has
taken x-rays, you may bring them with you to the
consultations. You may request that they be sent to us
ahead of time. We frequently, need to take a special
x-ray called a Panorex to more carefully evaluate your
jaw. Please make sure you have the following with you on
your first visit:

A list of all medications
you are currently taking.

All medical and dental
insurance information (forms and cards).

Please alert us of any
medical conditions which could be of concern PRIOR to
surgery.

FINANCIAL
POLICIES
In an attempt to keep your medical
costs to a minimum, we have adopted the following policy
regarding payments and billing: If you do not have
insurance coverage, payment is due in full on the day of your
surgery. For your convenience we accept most major
credit cards. We also accept cash and personal
checks. There is a fee for a returned check.
Financing programs through our office and Capitol One credit
are available. Please contact us for more details on
these programs.
We are participating
members of most major medical and dental plans. As a
courtesy to you, our staff will check your insurance coverage
and estimate your responsibility for the procedures to be
performed. As this is not a guarantee of payment, we
ENCOURAGE you to know your insurance benefits or visit your
human resources department for assistance. Pre-estimates
are not normally done but if you request it, we will gladly
prepare the forms to be sent to your insurance carrier.
Irregardless of coverage, any balance over 90 days becomes the
patient's responsibility and it is up to you to work with your
insurance carrier to obtain payment. Please feel
free to contact our office for further information on our
financial policies.

patient information and medical history forms, fill them out
and bring them with you to your first appointment.
